It's using it here (Makefile.PL):
my %opts = (
...
    INC          => "-I$ENV{POSTGRES_HOME}/include ...
...
    LIBS         => ["-L$ENV{POSTGRES_HOME}/lib -lpq"],

);

So it wants to find the include files and the libraries.
