Can anyone point me to a "recipe" for setting the max_fsm_* parameters?
Something at the level of

 Run 'VACUUM VERBOSE your-biggest-table-here'
 Look at the nice number
 Calculate a value
 Stick it in for max_fsm_*

I'm running with the default values, which I know aren't good, but I
don't want to just stick '100000' in (as many have recommended) without
knowing why that number is better than, say, '150000'.

It doesn't seem so.  I think the problem is that the FSM was new in
7.2, and so people don't have enough experience with it yet.  The
advice you usually see is "experiment; and if you do, please post
some results".
