pgsql: Update copyright for 2016

Update copyright for 2016

On closer inspection, the reason copyright.pl was missing files is
that it is looking for 'Copyright (c)' and they had 'Copyright (C)'.
Fix that, and update a couple more that grepping for that revealed.
