You can already do this via the following (baroque, but idempotent)
method:

UPDATE   pg_catalog.pg_settings
SET   setting =       CASE WHEN 'foo' = ANY(string_to_array(setting, ','))       THEN setting       ELSE setting ||
',foo'      END
 
WHERE   name = 'search_path'
;
