Patch applied.  Thanks.  Your documentation changes can be viewed in
five minutes using links on the developer's page,
http://www.postgresql.org/developer/testing.

I know I have fixed previous versions that did not use ampersand
escapes, but forgot to do it in 8.1.  I have added a documentation
mention that this needs to be adjusted for every release.
